Primary Care Team

The Sioux Lookout Area Primary Care Team (SLAPCT) is a mobile and collaborative team that provides communities with comprehensive primary health care services close to home.   

The Primary Care Team provides services to all age groups, with a specific focus on children and youth, preventative care and improved management of chronic disease (through both treatment and monitoring), as well as support for clients in improving self-management skills.

Services Offered

In select Northern Communities & Sioux Lookout:

Speech Language Pathology

Work with children and adults with speech language, communication, and swallowing disorders. SLPs can also address issues with feeding, vocabulary, grammar, reading, writing, social communication skills, and word finding skills after brain injury. 

Kinesiology

If you have joint pain, arthritis, diabetes, heart disease/stroke, or breathing problems they can help by providing safe and effective exercise programs and management advice.

Hepatitis C Treatment

A community-based program that provides Hep C support & treatment services to Sioux Lookout and SLFNHA’s 33 First Nation communities. The Hep C team consists of two Nurse Practitioners and a Registered Nurse.

Pharmacy Consultation

The Pharmacist can meet with patients who want to gain a better understanding of their current medications.

Nurse Practitioner

Provide support to other health care providers within the Primary Care Team.

Dietitian Consultation

Encourage patients, clients, and communities to embrace food, understand it, enjoy it. They can explain the science of nutrition in words everyone can understand and will sometimes organize presentations, cooking classes, and workshops in nursing stations, schools, and in the community.

Occupational Therapy

Work with anyone who is having difficulty with  day-to-day activities, such as showering or cooking. They may prescribe assistive devices like a raised toilet seat, recommend an easier way of doing activities, teach a caregiver a safer way of helping someone, or set up home modifications.

Physiotherapy

If you have sustained an injury or have a chronic health condition affecting how you function, they will assess you, provide education, provide a treatment plan, prescribe exercises, and help get you assistive devices and braces as needed.

Community Health Navigator

Works together with schools, community members, nursing stations, and other health organizations to connect them to PCT services.

Smoking Cessation

The Quit Smoking program at PCT provides you with one-on-one support during your attempt to quit. We provide a non-judgmental and supportive environment where we tailor a “quit plan” to meet your individual needs.

RPN - Foot Care

Our nurses provide lower limb assessments and diabetic foot exams, as well as performing nail care, callous paring, and other interventions.

Capital Needs

In addition, SLFNHA has also received a capital planning grant to support the capital needs associated with providing the above health care services in Sioux Lookout and in First Nation communities. To support this process, SLFNHA has identified a Capital Steering Committee. This Steering Committee has members that lead the process of development of a business plan and a capital plan, and assist in identifying the primary care needs of each community. These also include various approaches such as new builds or retrofits of existing buildings.
