Under the Sioux Lookout Area Primary Care Team (SLAPCT) and within the scope of practice of a Nurse Practitioner (NP), the NP will be responsible for providing primary health care to clients within the Sioux Lookout area. The NP assesses and diagnoses client health for acute illnesses and supports clients to manage chronic illnesses. The NP works with the client and the community to promote healthy lifestyles and disease prevention. The NP collaborates and consults with members of an interdisciplinary team to address issues experienced by clients and within the community and situations that are outside their scope of practice or exceeds their knowledge and/or skill.

The NP will provide holistic and integrated care to all ages, but with a focus on children and youth.  Care will be provided in the following settings:

  • To community members from the 33 First Nations communities accessing health services in Sioux Lookout
  • To the residents of the Sioux Lookout area
  • Mobile services into the 33 First Nations communities to which SLFNHA provides primary care services.
  • Act as a resource to all 33 Communities within the SLFNHA catchment.

The NP will:

  • Provide holistic care to an individual clients or groups of clients including family members and significant others.
  • Develop goals and objectives for client services and clinics.
  • Initiate programs in response to needs and new and existing legislation.
  • Review departmental performance in relation to goals, implement changes to improve quality of service
  • Independently and collaboratively with other health care providers assess, plan, implement and evaluate the individual care requirements of the client.
  • Ensure compliance with regulatory and legal requirements.
  • Manage expenses and submit statistical record required by the Ministry of Health.

The Nurse Practitioner will work out of the Northern Appointment Clinic will be directly accountable to the Director of Physician Services.

Minimum Qualifications:

  • Current registration as a Registered Nurse in the extended class, with the College of Nurses of Ontario and the Nurse Practitioner Association of Ontario
  • Nurse Practitioner Certificate from an accredited program.
  • Baccalaureate or Masters’ degree in nursing from a recognized university
  • 3 – 5 years experience working as a Nurse Practitioner in a community setting, hospital or public health setting with experience in primary health care preferred; new graduates are welcome to apply
  • Experience working with all age groups (newborn, children and youth, adults)
  • Awareness of and sensitivity to the health issues of a diverse community and experience working with First Nations communities.
  • Knowledge and understanding of infection control and preventative practices
  • Demonstrated commitment to and knowledge of community-based health care
  • Demonstrated ability to work effectively in a multi-disciplinary environment
  • Ability to provide coaching, mentorship, and guidance to community-based health workers.
  • Excellent Interpersonal skills including ability to coach and develop others
  • Excellent relationship-building skills to initiate and carry out initiatives, and build and maintain relationships with senior management, and internal and external stakeholders
  • Excellent verbal and written communication skills
  • Knowledge of the principles of collaborative leadership
  • The ability to function effectively during periods of rapid change and transition
  • Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an asset.

Roles and Responsibilities:

1) Assess and Diagnose Client Health
• Provide comprehensive primary health care to clients including assessment, diagnosis, treatment, referral for specialists or out-of-scope circumstances, and continuity of care and support services.
• Order and interpret investigative screenings and tests to aid in diagnosis and treatment
• Provide counselling, education and follow-up with clients to treat and manage illnesses
• Determine the presence of existing and potential health problems with a focus on lifestyle, psychological, socio-cultural and environmental risk factors that may influence the client’s health status
• Complete client health record within the electronic medical records database in an accurate and timely manner
• Collaborate and consult with physicians/specialists to address circumstances outside of their scope of practice or exceeding their knowledge and skill.
• Will deliver Primary Health Care according to the College of Nurses of Ontario Practice Standard for Registered Nurses in the Extended Class and the SLAPCT Strategic Directions.
• Collaborates with other SLAPCT members and health care providers to ensure management, referrals and information sharing to maintain the client’s continuity of care.
• Responsible for making clinical decisions, exercising judgment to ensure best practice nursing care is provided within available resources.
• Maintains confidentiality in the work place and within the community.

2) Support the Management of Chronic Illness in Clients
• Monitor the ongoing therapy of clients with chronic, stable illness by coordinating pharmacological, complementary and counselling interventions
• Prescribe therapies to manage the chronic illness
• Collaborate with physician services, Approaches to Community Wellbeing, and other areas to ensure a continuum of care/services

3) Work with clients and communities to promote healthy lifestyles, disease prevention and community health
• Support clients and their families to take increased responsibility for their health by involving them in risk factor and health problem identification, goal setting and the choice of interventions for disease treatment, intervention, prevention and health promotion
• Implement strategies to promote health and prevent disease with individuals, families, groups and communities.
• Synthesize information from individual clients to identify broader implications for health within the family and the community.
• Collaborate with physician services, Approaches to Community Wellbeing (Public Health), and other areas to develop comprehensive strategies for health promotion, disease prevention and community health.

4) Performs administrative duties in support of the organization’s goals and objectives.
• Participate in and promote the interdisciplinary provider model
• Provide consultation services to community groups, agencies and staff.
• Assist in the development and evaluation of policies, protocols and procedures to improve services to clients and to promote cooperative and efficient staff communications
• Assist in the development, planning and evaluation of treatment, education, counselling and health promotion programs
• Prepare statistical reports for reporting and evaluation purposes
• Is knowledgeable and participates in the Quality Management Program and Risk Management activities

5) Perform other duties as assigned within the scope of the position
• Engage in professional development to maintain current skills and knowledge to work to the scope of the position
• Assist in the development and implementation of the organization’s quality improvement initiatives
• Network with partner agencies to improve practices, enhance services and move forward with shared initiatives relevant to the position.
